Plaque Application

Please complete the form to the best of your ability.  If you are not sure of an answer please enter “unknown.” All fields are required with the exception of the “comments” field. You will be contacted for confirmation and payment. Payment is due is due at the time your plaque is ordered.

As of September 1, 2026, the fee for a basic plaque is $150, or $200 if the plaque is installed on a steel post (most plaques are installed this way unless there is no room in your yard). The installation and all materials are included in the price. We will install all plaques.
